Mobile device app

If you are on the go and away from your computer, you can still access Celgene Patient Support® with the Successful Start App. This is an app that you can download right onto your mobile device or tablet.

With the Successful Start App you can:

  1. Begin the enrollment process for your patients.
  2. Request a phone call or e-mail from your Specialist.
  3. Find and send information about support available for the Celgene medication you are prescribing, including patient financial brochures and other insurance-related services.*
  4. Learn who your Celgene Patient Support® Specialist is by entering the zip code of where your office is located.

Downloading the Successful Start App is easy:

  1. Scan the QR code with your mobile device
  2. Search “Celgene Start” in the iTunes App Store or Google Play. The Successful Start App is available for iPhone, iPad, and Android devices.

REMS programs

To avoid embryo-fetal exposure, REVLIMID® (lenalidomide), POMALYST® (pomalidomide), and THALOMID® (thalidomide) are only available through restricted distribution programs called Risk Evaluation and Mitigation Strategies (REMS). REVLIMID REMS® (formerly known as the RevAssist® program), POMALYST REMS®, and THALOMID REMS® (formerly known as the S.T.E.P.S.® program) require prescribers and pharmacies to be certified and patients to enroll and comply with all of the program requirements.

Fast Track for First Prescription®

Fast Track for First Prescription® is an optional service designed to support your practice and help patients receive their first prescription faster.

To be eligible, patients must:

  • Have received their first prescription for a Celgene oral oncology product
  • Have documented proof of insurance
  • Be enrolled in a Celgene REMS program

To enroll your patients in the Fast Track program, please provide Celgene Patient Support® with the following information:

  • Patient’s insurance information
  • Prescription
  • Patient’s Celgene REMS authorization number

To enroll a patient in Fast Track for First Prescription®, complete the online application and select the “Fast Track” option. Or call Celgene Patient Support® at 1-800-931-8691.

You may also fax a completed application to 1-800-822-2496 and note “Fast Track” on the fax.

Find a network pharmacy

Certain Celgene products are available exclusively through a select network of pharmacies due to restricted distribution programs.

Your Celgene Patient Support® Specialist can help determine which pharmacy is in network with your patient’s insurer.

Insurance forms

Letter of Medical Necessity
A letter of medical necessity is a letter written to the patient’s insurance company, requesting prior approval for medication or reconsideration of a denial. The letter should generally contain information on the patient’s medical history and current treatment plan. You can download guidelines for writing a letter of medical necessity here.

Appointment of Representative Release Form
The Appointment of Representative Release Form is often required by insurance companies in the event of an appeal. This form allows Celgene Patient Support® to appeal a denial on behalf of the patient. You can download the form here.
