Co-Pay and Other Assistance

If your patient cannot afford his/her medication, there are programs and foundations that may be able to assist them in paying for
their co-payments and out-of-pocket expenses. Your Celgene Patient Support® Specialist can assist you in identifying financial assistance
for your patients to ensure they get the Celgene medication you have prescribed.

Celgene Free Medication Program (Patient Assistance Program or PAP)
If your patient is unable to pay for a Celgene medication that you have prescribed, they may be eligible for the Celgene free medication program.
The Celgene free medication program offers free medication to patients who otherwise cannot afford their medications. Celgene has contributed
nearly $550 million in free medication over the past 10 years.
Patients must meet financial criteria to be eligible for assistance. Eligibility is based on a patient's household income and assets, as
described below:
